Nattokinase is an enzyme extracted and purified from the Japanese food product natto. Natto is produced by fermenting cooked soybeans with the bacterium Bacillus subtilis natto (which also produces this enzyme). While other soy foods contain enzymes, only natto preparations contain the specific nattokinase regulated by the Japanese Nattokinase Authority and the Ministry of Health, Labor and Welfare. Despite its name, nattokinase is not a kinase, but rather a serine protease from the subtilisin family.

Nattokinase can directly or indirectly break down fibrin (the main component of blood clots) in the blood, inhibiting excessive platelet aggregation and thus reducing the risk of blood vessel blockage. Research suggests that it may enhance the body’s natural thrombolytic capacity by activating the conversion of plasminogen to plasmin. However, its effectiveness is not yet a substitute for medication and should be used with caution in patients with established cardiovascular disease.
By reducing blood viscosity and promoting microcirculation, nattokinase may alleviate symptoms such as cold hands and feet and fatigue caused by poor blood flow. Experimental studies have shown that long-term use may improve blood flow, but this should be combined with diet and exercise.
Some clinical trials suggest that nattokinase may help lower blood pressure by dilating blood vessels and reducing angiotensin production. For example, a study of patients with hypertension found that continued intake slightly reduced systolic and diastolic blood pressure, but the effect was weaker than that of conventional antihypertensive medications.
Animal studies and a limited number of human studies suggest that nattokinase may reduce the oxidation of low-density lipoprotein (LDL), also known as “bad cholesterol,” and inhibit the formation of atherosclerotic plaques. However, large-scale data support its lipid-lowering effects, and its sole use for the treatment of hyperlipidemia is not recommended.
The active ingredients in nattokinase may mitigate endothelial damage by scavenging free radicals and inhibiting the release of inflammatory factors. This property is believed to have potential benefits for chronic inflammatory diseases such as atherosclerosis, but the mechanism is not fully understood.
When taking nattokinase, it is best to follow the manufacturer’s recommendation. In most cases, they recommend 2.000 FU daily – a dosage that studies show is effective. However, this recommendation does NOT apply to people who take blood thinners or those who want a dosage tailored to their needs. These people should seek individualised advice from their doctor and, if necessary, determine the appropriate dosage with the help of medical expertise.
